Certification

Admission Requirements

• Be a minimum of 16 years of age to be certified.
• Be in good health and physical condition
• Provide photographic identification to the Evaluator prior to issuance of a certificate
• Complete the agency registration forms as well as exam application forms prior to course commencement. The forms must be completed entirely.
• Provide a parent or guardian signature if under age 19.
